As an Advanced Practice Provider, you’ll play a vital role in delivering proactive, relationship-based care in skilled nursing and long-term care facilities. With personalized 1-on-1 Clinical Team Leader mentorship, you’ll be able to support patient goals, reduce avoidable patient transfers, and lead with compassion.
Graduate from accredited Nurse Practitioner Program or accredited Physician Assistant Program
Older Adult or Family Nurse Practitioner specialty (other concentrations considered)
Minimum 2 years’ experience with adult/geriatric patients (preferred – new graduates welcome in specified locations)
Unrestricted APRN license or PA-C license and DEA registration for practice state(s)
